Our Referral Process
A simple, streamlined process designed for efficiency and clarity.
Initial Contact
Call or email our referral team with basic details about the individual and their support needs.
Referral Form
Complete our simple referral form (available to download below) with relevant background information.
Assessment
We conduct a needs assessment, often within 24-48 hours, to ensure we can meet the individual’s requirements.
Placement
If suitable, we arrange a move-in date and introduce the individual to their support worker and accommodation.
Ongoing Communication
Regular updates, multi-agency meetings, and collaborative review processes to ensure positive outcomes.
Eligibility & Suitability
We Support Adults (18+) With:
- Mental health needs (including dual diagnosis)
- Offending history and probation requirements
- Substance misuse (in recovery or willing to engage)
- Complex trauma backgrounds
- Homelessness or housing instability
- Limited independent living skills
